What is nifedical XL?

Nifedical XL is in a group of drugs called calcium channel blockers. It works by relaxing the muscles in the heart and blood vessels. Nifedical is used to treat hypertension (high blood pressure) and angina (chest pain).

How do I take nifedical XL?

Nifedical comes in a capsule and an extended-release (XL) tablet to be taken mouth. Typically, the capsule is taken three or four times daily. The extended-release tablet should be taken once daily on an empty stomach, either 1 hour before or 2 hours after a meal around the same time every day. Nifedical should be taken as directed by your prescribing doctor. Do not crush, chew, or break extended release tablets.

Before taking nifedical XL, tell your doctor if:

  • You are allergic to nifedical, any other medications, or any of the ingredients in nifedical
  • You are taking any other prescription and nonprescription medications, vitamins, nutritional supplements, specifically if you are taking any of the following: acarbose, blood thinners, antifungals, beta blockers, carbamazepine, cimetidine, digoxin, diltiazem, doxazosin, erythromycin, fentanyl, or any HIV protease inhibitors
  • If you are taking any herbal products, especially St. John's wort
  • If have or have ever had a narrowing or blockage of your digestive system or any other condition that causes food to move through your digestive system more slowly; or heart, liver, or kidney disease
  • If you have had a myocardial infarction within the last 2 weeks
  • If you are pregnant, plan to become pregnant, or are breast-feeding
  • If you are having surgery, including dental surgery
  • If you have/suffer from any of the following: heart disease, diabetes, low blood pressure, gastroesophageal reflux disease, an underactive thyroid, aortic stenosis, colon cancer, or constipation
  • If you have had a gastric bypass or gastric banding surgery, a section of your bowel surgically removed, or a colostomy 

What are the possible side effects of taking nifedical XL?

Some serious side effects that require contacting your doctor immediately include: swelling of the face, eyes, lips, tongue, hands, arms, feet, ankles, or lower legs, difficulty breathing or swallowing, fainting, rash, yellowing of the skin or eyes, or an increase in frequency or severity of chest pain.

More common side effects that still should be mentioned to your doctor: headache, nausea, dizziness or lightheadedness, feeling warm or flushed, heartburn, rapid heartbeat, muscle cramps, constipation, cough, or decreased sexual ability.

 

What if I forget to take a dose of nifedical XL?

If you miss a dose of nifedical, take it as soon as you remember it. However, if it is almost time for the next dose, continue with your regular dosing schedule. Do not take a double the amount to make up for a missed dose. 

How do I store nifedical XL?

Keep nifedical in the container it came in, closed tightly, and out of reach of children. Store it at room temperature, away from light, and away from excess heat and moisture. Discard any medication that is outdated or no longer needed.

 

What happens if I overdose on nifedical XL?

In case of overdose, call your local poison control center at 1-800-222-1222. If the victim has collapsed or is not breathing, call local emergency services at 911.

Symptoms of a nifedical overdose may include: dizziness, rapid heartbeat, flushed feeling, nervousness, nausea, vomiting, swelling of the hands, feet, ankles, or lower legs, blurred vision, or fainting.

Warnings

Grapefruit and grapefruit juice may interact with nifedical and cause unwanted side effects. Avoid getting up too fast from a sitting or lying position, or you may feel dizzy while taking nifedical. Nifedical may cause hypotension, therefore it is recommended to monitor BP initially when starting this medication. It also may increase frequency, duration, and/or severity of angina (chest pain).

Brand names for nifedical include Adalat, Adalat CC, Afeditab CR, Nifedical XL, Nifeditab CR, Procardia, and Procardia XL.
