What is allopurinol?

Allopurinol is used in the treatment of gout. It also helps treat kidney stones, and decreases levels of uric acid in individuals undergoing treatment for cancer. In a class of medications called xanthine oxidase inhibitors, allopurinol helps to eliminate uric acid and prevents the onset of kidney stones and gout.

How do I take allopurinol?

Allopurinol should be taken orally and comes in tablet form. Typically, the medication is taken once or twice a day, after food and around the same time daily. Take allopurinol exactly as directed by your prescriber.

Your doctor may start you on a low dose and increase your dose as needed.

Allopurinol can take several months for users to fully feel the effects. An increase in gout attacks may occur over the first few months, but will eventually prevent them. You may be asked to take a supplemental medication to help prevent gout attacks while waiting for allopurinol to fully set in. Do not discontinue allopurinol without consulting with your doctor first. 

Before taking allopurinol, tell your doctor if:

  • You are allergic to allopurinol or any other medications
  • You are taking any other prescription and nonprescription medications, vitamins, herbal products, or nutritional supplements. Specifically mention if you are taking the following: amoxicillin, ampicillin, blood thinners, chemotherapy drugs, mercaptopurine, chlorpropamide, diuretics, azathioprine, cyclosporine, other medications for gout like probenecid,  sulfinpyrazone, or tolbutamide
  • You have a history of kidney disease
  • You have a history of liver disease
  • You have a history of heart failure
  • You are pregnant, plan to become pregnant, or are breast-feeding

What are the possible side effects of taking allopurinol?

Common allopurinol side effects that should be mentioned to your doctor include: Stomach upset, nausea, diarrhea, or drowsiness.

Less common, more serious allopurinol side effects that require contacting your doctor immediately include: numbness/tingling of arms or legs, yellowing of the skin or eyes, severe stomach pain, persistent nausea/vomiting, weight loss, pain in the eyes, or vision change, bleeding/bruising, signs of infection like fever, unusual fatigue, painful/bloody urination, change in the color or frequency of urine.

Signs of an allergic reaction that require immediate medical attention include:  swelling of the tongue, face, or throat, rash, itching, dizziness, or difficulty breathing. 

What if I forget to take a dose of allopurinol?

If a dose of allopurinol is missed, administer it as soon as you recall. If it is just about time for the next dose, do not double up on doses, skip your missed dose and continue with your regular dosing schedule. 

How do I store allopurinol?

Allopurinol should be kept out of reach of children. The medication is best kept its original packaging or bottle. It should be kept in a dry, cool area, away from any moisture.  If the medication has expired or is no longer needed, it should be discarded.

What happens if I overdose on allopurinol?

If an overdose or an allergic reaction is suspected, contact your local poison control center at 1-800-222-1222, call 911, or go to an emergency room immediately.

Warnings

Allopurinol can cause drowsiness. Avoid driving, using machinery, or performing any activity that requires a high level of alertness until you have determined how you respond to the medication.

Allopurinol may lower blood cells that help fight infections, making it easier for you to bleed, bruise, or become ill.

To reduce the risk of kidney stones forming, drink 8 to 10 full glasses of fluid every day and try to drink a full glass when taking allopurinol. Also limit alcohol consumption while taking allopurinol. It can decrease the effectiveness.
