What is cyanocobalamin?

Cyanocobalamin is a manufactured form of vitamin B12, a nutrient essential for healthy growth of red blood cells, proteins, and nerve cells. It is prescribed to treat and prevent vitamin B12 deficiencies resulting from infections, a vegan diet, certain medications, or pernicious anemia. It may also be used to test a body’s ability to absorb B12. Cyanocobalamin may also be prescribed to treat other conditions, including methylmalonic aciduria as well as inherited conditions that prevent intestinal absorption of vitamin B12.

How do I take cyanocobalamin?

Cyanocobalamin is administered via subcutaneous or intramuscular injection in a medical setting. The initial treatment consists of daily injections for the first week; as red blood cell levels return to normal, injection frequency will be gradually reduced until they are administered on a monthly basis. Stopping this medication will result in the return of anemia and will cause nerve damage. Other forms of cyanocobalamin, pills, lozenges, or nasal spray, are available as generic or brand name drugs or in combination with other medications; however, do not take these as a substitute for injections of the cyanocobalamin drug unless approved by your healthcare provider. Follow the dosing instructions that accompany these forms of cyanocobalamin.

Before taking cyanocobalamin, tell your doctor if:

  • You are allergic to any hydroxocobalamin, cobalt, any medications, or any ingredients in the cyanocobalamin medication
  • You are pregnant, planning a pregnancy, or are nursing a child
  • You are taking any other medications, vitamins, or supplements
  • You have a folic acid or iron deficiency
  • You have Lebers optic neuropathy
  • You are receiving bone marrow treatment or medication
  • You have kidney or liver disease
  • You regularly consume alcohol

What are the possible side effects of taking cyanocobalamin?

Cyanocobalamin may cause mild side effects, including diarrhea or feeling swollen, but these should subside. If they persist or worsen, contact your doctor. Serious side effects have also been reported; seek immediate medical help if you experience any of the following: arrhythmia; disorientation; coughing; wheezing; dizziness; extreme fatigue; extreme thirst; increased urination; headache; leg pain or muscle weakness, cramps, or pain; pain, warmth, redness, swelling or tenderness in one leg; red skin color, especially on the face; shortness of breath; swelling of the extremities; tachycardia.

What if I forget to take a dose of cyanocobalamin?

If you miss your scheduled injection of the cyanocobalamin drug, contact your doctor immediately to reschedule. If you are on a dosing regimen of cyanocobalamin pills, nasal spray, capsules, or lozenges, take the missed dose as soon as you remember, unless it is almost time for the next one; in this situation, skip the one missed and return to the regular schedule. Do not increase the amount taken or used to compensate for a missed dose.

How do I store cyanocobalamin?

Injectable cyanocobalamin will be stored in your doctor’s office. Oral or nasal formulations should be kept in their original containers, out of the reach of children in an environment away from the extremes of heat and humidity. Do not allow the nasal spray to freeze. Ask your pharmacist about the proper method of disposal for any expired or unused expired medication.

What happens if I overdose on cyanocobalamin?

Contact the Poison Control Center at 1-800-222-1222 if an overdose of cyanocobalamin occurs. If the individual has stopped breathing or lost consciousness, contact Emergency Services at 911.

Warnings

Limit alcohol consumption while taking cyanocobalamin. This medication may need to be taken on a permanent basis. Contact your doctor if you become pregnant while taking cyanocobalamin.

 

Brand Names

Cyanocobalamin is manufactured vitamin B12; a nasal spray version is marketed under the brand name Nascobal. However, these cannot be substituted for each other without written medical permission.
