What is Nortriptyline HCL?

Nortriptyline HCL is used to restore balance in certain natural substances within the brain that are necessary to maintain mental balance. Nortriptyline HCL is also sometimes used to treat panic disorders and post-herpetic neuralgia (the burning, stabbing pains or aches that may last for months or years after a shingles infection). Nortriptyline HCL has also been used to help people stop smoking and may be used for other treatments. Nortriptyline hydrochloride is a tricyclic antidepressant and it works by inhibiting the neurotransmission of such diverse agents as histamine, 5-hydroxytryptamine, and acetylcholine. 

How do I take Nortriptyline HCL?

Nortriptyline HCL is available in both capsule and liquid form, which are both taken by mouth. It is typically dosed four times per day and may be taken with or without food. Each dose should be taken around the same time every day. Nortriptyline HCL should be taken exactly as directed by your health care professional, and you should ask your doctor or pharmacist to explain any part of the directions which you do not understand. Do not take nortriptyline HCL more often than prescribed.

Your healthcare professional will likely start you on a low dose of nortriptyline HCL and gradually increase your dose. Continue taking nortriptyline HCL even if you feel well. Never stop taking nortriptyline HCL without first talking to your doctor. If you suddenly stop taking nortriptyline HCL, you may experience withdrawal symptoms such as headache, nausea, and weakness. 

Before taking nortriptyline HCL, tell your doctor if:

  • You are allergic to nortriptyline and other tricyclic antidepressants.
  • You are taking a monoamine oxidase inhibitor or if you have stopped taking an MAO inhibitor within the past 14 days.
  • You are taking any prescription or non-prescription medications, vitamins, or nutritional supplements.
  • You have recently had a heart attack.
  • You have or ever had glaucoma, an enlarged prostate, difficulty urinating, diabetes, seizures, schizophrenia, an overactive thyroid gland, or liver, kidney or heart disease.
  • You are pregnant, plan to become pregnant, or are breastfeeding. If you become pregnant while taking Nortriptyline HCL, call your doctor.
  • You are having surgery, including dental surgery.

What are the possible side effects of taking Nortriptyline HCL?

Seek emergency medical treatment if, after taking Nortriptyline HCL, you experience irregular heartbeat, jaw, neck, and back muscle spasms, slow or difficult speech, shuffling walk, uncontrollable shaking of a part of the body, fever, difficulty breathing or swallowing, rash, or yellowing of the skin or eyes.

Additional but less-serious side effects you should tell your doctor include nausea, drowsiness, weakness or tiredness, excitement or anxiety, nightmares, dry mouth, changes in appetite or weight, constipation, difficulty urinating, frequent urination, blurred vision, changes in sex drive or ability, or excessive sweating.

Nortriptyline HCL may cause other side effects. Call your doctor if you have any unusual problems while you are taking this medication.  If you experience serious side effects, you or your doctor may send a report to the Food and Drug Administration’s (FDA) MedWatch Adverse Event Reporting program online at http://www.fda.gov/Safety/MedWatch or by phone at 1-800-332-1088.

What if I forget to take a dose of Nortriptyline HCL?

If you miss a dose of nortriptyline HCL, take it as soon as you remember it. If it is almost time for the next dose, skip the missed dose and continue your regular dosing schedule. Do not take a double dose to make up for the missed one.

How do I store Nortriptyline HCL?

Nortriptyline HCL should be stored in the container that it came in and out of the reach of children. Store it at room temperature, away from excess heat and moisture (not in the bathroom). Any unused medication that is outdated or no longer needed should be thrown away. Your doctor or pharmacist can advise you about proper disposal of your medication.

What happens if I overdose on Nortriptyline HCL?

If overdose is suspected, contact a poison control center or emergency room immediately. US residents can call their local poison control center at 1-800-222-1222, and Canadian residents can call a provincial poison control center.  If the patient has collapsed or is not breathing, call local emergency services at 911.

Symptoms of nortriptyline HCL overdose may include irregular heartbeat, seizures, coma, confusion, hallucination, widened pupils, drowsiness, agitation, fever, low body temperature, stiff muscles, and vomiting.

Warnings

You should not take nortriptyline HCL if you have recently had a heart attack, if you are allergic to nortriptyline or to similar antidepressants such as amitriptyline, amoxapine, clomipramine, desipramine, doxepin, imipramine, protriptyline, or trimipramine, or if you are being treated with methylene blue injection. Do not use nortriptyline HCL if you have taken an MAO inhibitor within the last 14 days. Some young people have thoughts about suicide when first taking an antidepressant. Your doctor will need to check your progress at regular visits while you are using nortriptyline HCL. Your family or other caregivers should also be alert to changes in your mood or symptoms. Report any changes, such as mood or behavior changes, anxiety, panic attacks, trouble sleeping, or if you feel impulsive, irritable, agitated, hostile, aggressive, restless, hyperactive (mentally or physically) more depressed, or have thoughts about suicide or hurting yourself.
