What is pramipexole dihydrochloride?

Sometimes referred to simply as pramipexole, pramipexole dihydrochloride is the generic version of Mirapex (either standard release or extended release). It is a non-ergot dopamine agonist, meaning that it improves body tremors, stiffness, motion control, and balance by acting like natural dopamine in the brain. The precise method of action for pramipexole dihydrochloride is unknown. It is especially good for early stage, idiopathic Parkinson's Disease, although it is also prescribed to severe, late stage patients who are also taking Levodopa. Please note: pramipexole dihydrochloride is meant only to treat a particular set of symptoms, and is not a cure for Parkinson's or for any other disorder your doctor may prescribe it for.

How do I take pramipexole dihydrochloride?

The dosage prescribed by your doctor is tailored to fit your needs and your specific therapeutic response and tolerability, so always follow your doctor's instructions specifically. Your doctor will start your doses at sub-therapeutic levels and step up your dosage over time.  In most Parkinson's cases, it involves taking 1 pill 3 times daily of the normal release or 1 pill 1 time daily for the extended release. The tablets are taken orally, and taking them with food may diminish nausea. Always swallow the pill whole, and do not crush, break, cut or chew your pills. Always take the amount prescribed, not more or less. If you have any questions, refer to the included pamphlet or talk to your doctor or pharmacist.

Before taking pramipexole dihydrochloride, tell your doctor if:

  • You have any medical or food allergies, as there may be inactive ingredients you could have a reaction to.
  • You’re taking any other medications that include pramipexole.
  • You’re taking other prescription or non-prescription medication, supplements, or vitamins in order to avoid drug interactions. This is especially important for any other Parkinson's medications you may be taking (e.g., Levodopa), antidepressants, anti-anxiety medications, seizure or other neurologic medications or digestive aids.
  • You are pregnant, plan to become pregnant, or are breastfeeding.
  • You have recently had surgery or are planning to have surgery, even dental surgery. Be sure to tell your surgeon that you're taking pramipexole.
  • You regularly drink alcohol.
  • You have urges that are hard to control, such as gambling or hypersexuality, as impulse control disorders can be made worse by this medication.
  • You have been diagnosed with any sleep disorders (not including RLS).
  • You have a history of or currently have heart or kidney disease.
  • You have low blood pressure and/or experience lightheadedness or dizziness (especially while standing up), fainting, or daytime sleepiness.

What are the possible side effects of taking pramipexole dihydrochloride?

Some side effects can be severe, and you should call your doctor immediately. These include but are not limited to: hallucinations; dark and cloudy, cola-colored, or bloody urine; increased or frequent urination, loss of bladder control; difficult or painful urination;  unusual body spasms (e.g., twisting, writhing); sleep attacks and irresistible drowsiness; sudden changes in thought (e.g. paranoia  or fearfulness); changes in vision (e.g. double vision); unusual muscle weakness or difficulty swallowing; difficulty remembering things, focusing, or concentrating; chest tightness or unusual difficulty breathing; swelling in various parts of the body (especially in the arms and legs).

Common side effects include: loss of appetite and weight loss, indigestion and heartburn, constipation, dry mouth, headaches, general discomfort, increased coughing and runny nose, increased sweating, rashes or itching, decreased sexual drive or ability to function sexually, abnormal dreams or daydreams.

What if I forget to take a dose of pramipexole dihydrochloride?

Never take a double dose of pramipexole dihydrochloride to make up for a missed dose. If you're taking extended release pills, you may take your missed dose as soon as you remember it, unless it is almost time for your next dose. If you're taking standard release pills, simply skip the missed dose and maintain your regular dosing schedule.

If you have missed multiple doses in either form, call your doctor to see how you should proceed.

How do I store pramipexole dihydrochloride?

Store pramipexole dihydrochloride in a dry place at room temperature where it is protected from light. Always keep this medication out of reach from children and pets.

What happens if I overdose on pramipexole dihydrochloride?

While there is no clinical study that found overdosing related to pramipexole dihydrochloride, if an overdose is suspected, call your local poison control center or seek medical attention immediately.

Warnings

Some people have reported having sleep attacks (i.e., falling asleep while alert and doing tasks such as driving) while taking pramipexole dihydrochloride. Learn how you react to your medication before driving, operating machinery, or doing any other potentially hazardous activities.

Additionally, pramipexole may exacerbate impulse control disorders associated to Parkinson's Disease. If you notice increased urges, or new or unusual urges (e.g., to gamble, hypersexuality), talk to your doctor immediately.

There have not been studies regarding taking pramipexole during pregnancy, so if you become pregnant while taking it, talk to your doctor about how to proceed. They may want to adjust your medications.
