What is promethazine/codeine?
Promethazine with codeine is a combination of an antihistamine (promethazine) and a narcotic (codeine) that is used to treat cough, cold, and allergy symptoms. Promethazine blocks histamines to reduce inflammation, stop vasodilation, and stop allergy symptoms. Promethazine has a sedative effect. Codeine is an opiate drug that also has a sedative effect and stops coughing. The combination of promethazine and codeine is commonly abused and taken recreationally. This combination is especially dangerous when mixed with alcohol, as all three of these ingredients cause the depression of the central nervous system. There have been several deaths in the United States, including celebrity deaths, due to the combination of this medication with alcohol. Use this medication with caution around those who are prone to drug abuse or alcoholism.
How do I take promethazine/codeine?
Promethazine/codeine comes as a syrup to be taken orally. Use a medicine measuring device to measure your dosage before taking the medication. Do not use an ordinary household spoon. If you don’t have a medicine measuring cup, get one from your pharmacist. Swallow the medication, and then drink a glass of water. If this medication upsets your stomach, eat food or drink a glass of milk before taking this medicine. Take doses at regular intervals as prescribed by your doctor.
Before taking promethazine/codeine, tell your doctor if:
- You are pregnant or breast feeding.
- You are taking any prescription or nonprescription medications or herbal supplements, especially barbiturates, narcotics, sodium oxybate, antidepressants, haloperidol, anticholinergics, and HIV medications.
- You are allergic to any of the ingredients in this medication or you have diabetes.
- You have a chronic cough or breathing problems.
- You have a history of heart problems, low blood pressure, or blood or bone marrow problems.
- You have suffered a head injury, had a brain tumor, or experienced any other infections of the brain or nervous system.
- You have an enlarged prostate or bladder problems.
- You have stomach ulcers, bowel problems, or chronic constipation.
- You have a history of drug or alcohol abuse.
Promethazine and codeine syrup should not be used to stop a productive cough, or a cough that is expelling mucous from the lungs. If you cough continues for more than five days or you develop a fever, contact your doctor. Avoid alcohol while taking this medication. Promethazine with codeine may make you more sensitive to sunburn. Use protective sunscreen and avoid the sun or tanning while taking this medication.
What are the possible side effects of taking promethazine/codeine?
Possible side effects from taking promethazine with codeine include dry mouth, nose and throat; dizziness, drowsiness, nausea, vomiting, headache, constipation, and weakness. In rare cases and in cases of misuse of this medication, a patient can develop neuroleptic malignant syndrome (NMS). If signs of fever, confusion, stiff muscles, problems with thinking or delirium, or irregular heartbeat occur, contact your doctor at once.
What if I forget to take a dose of promethazine/codeine?
If you miss a dose of promethazine/codeine, take the missed dose as soon as you remember. If it’s almost time for your next dose, skip the missed dose and continue on your regular schedule. Do not take a double dose of promethazine/codeine.
How do I store promethazine/codeine?
Store promethazine and codeine syrup at room temperature between 59 and 86 degrees Fahrenheit. Keep this medication away from light and out of reach of children. Keep it out of reach of anyone who might be prone to drug abuse.
What happens if I overdose on promethazine/codeine?
Signs of overdose on promethazine/codeine include depressed or difficult breathing, slowed heartbeat, dilated pupils, exaggerated reflexes or lack of motor skills, limp or stiff muscles, seizures, extreme dizziness and drowsiness, cold and clammy skin, and coma. Overdose on promethazine and codeine syrup can be fatal, especially when mixed with alcohol and other drugs. If you or someone you know has overdosed on promethazine/codeine, contact 911 emergency services.
Warnings
Do not drink alcohol or use sleeping medications or sedatives while taking promethazine and codeine. Side effects could be fatal. Do not give this medication to children under six years of age. Serious or fatal breathing problems could occur.
