What is Tobi?

Tobi, otherwise referred to as tobramycin, is an aminoglycoside antibiotic for treatment of Pseudomonas aeruginosa bacterial lung infections afflicting patients with cystic fibrosis. It's a slightly yellow, clear solution to be taken via nebulizer, and it's intended to be taken concurrently with your other cystic fibrosis medications.

How do I take Tobi?

Neither the capsules nor the solution are intended to be taken by mouth; always be sure to use the appropriate inhaler or nebulizer for your medication. If you are taking multiple inhaled medications, always take the Tobi drug last. Do not mix medications in your inhaler or use your Tobi inhaler for other medications. Always follow your doctor's or pharmacist's instructions, and read the instruction pamphlet included with your medication.

For capsules: Load 1 capsule into your Tobi Podhaler each time you take your dose. Press the appropriate buttons for the Podhaler to pierce the capsules, releasing the powder into the inhalation chamber. Sitting up straight, inhale deeply and slowly until the chamber is empty in order to receive your full dose.

For the solution: While the drug chamber is separated from your compressor, open 1 ampule and empty the solution into the chamber. Connect the chamber to the compressor, and put on your mask or mouthpiece. Turn on the compressor and breathe evenly and slowly until all you've emptied the drug chamber and all the mist is gone (about 10 to 15 minutes). This ensures you receive your full dose. Follow the cleaning instruction included with your nebulizer.

Before taking Tobi, tell your doctor if:

  • You have any food or medical allergies, or any other allergies.
  • You are pregnant, plan to become pregnant, or are breastfeeding.
  • You are taking Amifampridine, as this medication would interact with Tobi medicine dangerously.
  • You are taking any other prescription or non-prescription drugs, vitamins, or supplements, in order to avoid interactions.
  • You smoke or drink regularly.
  • You have a history of muscle weakness or have a neuromuscular disorder (e.g., Parkinson's Disease).
  • You have allergies to or a history of a bad reaction to other aminoglycoside antibiotics.
  • You have a history of or currently experience breathing or kidney problems or disease.

What are the possible side effects of taking Tobi?

If you experience any of the following side effects, seek emergency medical assistance immediately: vertigo or the constant sense of moving or the surroundings spinning; irregular or shallow breathing; lightheadedness or dizziness, severe drowsiness; blue nails, lips, or skin.

Other severe side effects may include: bloody nose or other unusual bleeding or bruising, difficulty breathing or shortness of breath, tightness or pain in the chest, coughing up unusually colored phlegm, fever or chills, sore throat, swollen glands, ulcers or white sores near the mouth, unusual tiredness or weakness, voice changes, difficult or painful urination, black or tarry stool. If you experience any of these effects, contact your doctor as soon as possible.

Common side effects from taking Tobi include general discomfort, hoarseness, change in or loss of taste, rash, back pain, diarrhea.

What if I forget to take a dose of Tobi?

You can take your dose as soon as you remember it, unless it is almost time for your next dose (6 hours or less). In that case, skip the missed dose. Always maintain your regular dosing schedule, and do not take two doses at the same time.

How do I store Tobi?

Store the capsule form at room temperature away from moisture and light, and only take a capsule out when you're ready to take your medication.

The solution should be stored in the refrigerator at about 36°F to 46°F. If that's not possible, Tobi solution should be stored at the coolest room temperature possible, and keep it protected from light. The solution may get darker at this temperature - it is still safe to use.

What happens if I overdose on Tobi?

Call your local poison control center or seek medical assistance immediately. Symptoms of an overdose may include: loss of high tone hearing or tinnitus, vertigo or dizziness, respiratory failure, neuromuscular blockade (paralysis), or renal failure.

Warnings

Do not take live-bacterial vaccines while on Tobi medicine, as it can interfere with their efficacy. If you need to take a vaccine, be sure to tell the administrating medical personnel that you're taking Tobi.

Tobi can affect the kidneys, so be careful about what other over-the-counter medications you take that can also be harmful to the kidneys (e.g., aspirin, ibuprofen, diuretics). If you already have impaired hearing or balance, or have other vestibular issues, Tobi may exacerbate these problems. Do not take your medication after its listed expiration date.

Brand Names

Unfortunately, the Tobi drug is not currently offered as a generic. Tobi medicine is so specific, the only other brand available is Bethkis.
