Spironolactone Information

What is spironolactone?

Spironolactone is a potassium-sparing diuretic that is primarily used to treat heart failure and fluid retention resulting from liver or kidney disease and ascites. It works by decreasing the reabsorption of sodium and water, and also decreases the secretion of potassium. Spironolactone is also used to treat high blood pressure and Conn’s syndrome. Besides acting as a diuretic, spironolactone also inhibits the function of androgens in the body which are steroid hormones that develop and regulate male sex characteristics. Testosterone is an example of an androgen. Due to its anti-androgen properties, it is used to treat acne and male pattern baldness and for hormone replacement therapy, breast development, and sex reassignment therapy for male to female changes.

How do I take spironolactone?

You can buy spironolactone at your local drugstore with a prescription from your doctor. Spironolactone comes as a tablet to be taken orally with a glass of water. This medication is usually prescribed to be taken once or twice daily. Follow instructions given to you by your doctor. Because this medication is a diuretic and causes urination, it is best to take it before 6 pm to avoid sleep disruption.

Before taking spironolactone, tell your doctor if:

  • You are taking any other prescription or nonprescription drugs, vitamins or supplements, especially eplerenone, Inspra, ACE inhibitors, angiotensin receptor blockers (ARBs), NSAIDs, barbiturates, blood pressure medications, steroids, potassium supplements, and diuretics.
  • You are allergic to spironolactone or any of the other ingredients in this medication.
  • You have or have ever had Addison’s disease, menstrual disease, or kidney disease.
  • You have high blood potassium levels.
  • You are pregnant, plan to become pregnant, or are breast feeding.
  • You have a history of heart problems or diabetes.
  • You are dehydrated or have low sodium levels.

This medication is to be used with caution in the elderly. Spironolactone causes raised potassium levels which may lead to irregular heartbeat that can be fatal. Monitor your potassium intake while taking this medication.

What are the possible side effects of taking spironolactone?

Spironolactone causes frequent urination, dizziness and drowsiness. Other side effects include dry skin, rashes, feminization in males, testicular atrophy, sexual dysfunction, drop in libido, menstrual irregularities, breast tenderness or enlargement, stomach pain or cramping, headache, increased hair growth, thirst or dry mouth, lack of coordination, irregular heartbeat, vomiting, hyperkalemia, diarrhea, blood in stools, and confusion. These are not all the possible side effects from taking spironolactone. Contact your doctor if side effects are severe or unusual.

What if I forget to take a dose of spironolactone?

If you miss a dose of spironolactone, take the missed dose as soon as you remember. If it’s almost time for your next dose, skip the missed dose and continue on your regular schedule. Do not take a double dose of spironolactone.

How do I store spironolactone?

Store spironolactone tightly sealed in the container it came in. Keep it at a room temperature below 77 degrees Fahrenheit. Keep this medication out of reach of children and pets.

What happens if I overdose on spironolactone?

An overdose of spironolactone can result in nausea and vomiting, confusion, diarrhea, irregular heartbeat, tingling or weakness in the legs, rash, dizziness, and skin that is cold, clammy or gray. If you or someone you know has overdosed on spironolactone, contact the Poison Control Center at 1-800-222-1222. If the victim is unconscious, contact 911 emergency services.

Warnings

Spironolactone has been shown to cause tumors in rats, although it hasn’t been proven to cause cancer in humans. Because of its anti-androgen properties, this medication may cause breast development and feminization in men. This medication is a diuretic and may cause dehydration. Ask your doctor how much water you should drink to stay hydrated.
