What is isotretinoin?

Isotretinoin is a retinoid medicine, which means that it is derived from vitamin A. Retinoids are normally used as topical medications, but oral isotretinoin is prescribed to treat severe cases of acne. Isotretinoin is a very strong medication that can be toxic in large doses. It works by increasing skin cell turnover and reducing sebum production. This medication should only be prescribed to treat severe cases of acne. How do I take isotretinoin?

Isotretinoin pills are normally taken twice daily at the same time every day. This medication should always be taken with food. Taking isotretinoin on an empty stomach makes it less effective. Your doctor may adjust your dosage over time depending on your body’s response. Your doctor should also monitor your liver function throughout your course of treatment. Be sure to keep all your lab appointments. A course of treatment normally takes 4-6 months. Acne may appear to get worse during the first couple of weeks of treatment as comedones come to the surface. After this, your skin should start to clear.

Before taking isotretinoin, tell your doctor if:

• You are pregnant, plan to become pregnant or you are breastfeeding
• You have a history of mental illness.
• You are allergic to any of the ingredients in isotretinoin pills.
• You have diabetes.
• You have osteoporosis.
• You have asthma.
• You have heart or liver disease.
• You are anorexic.
• You normally drink alcohol.
• You have high triglyceride or high cholesterol levels.
• You are an athlete or normally participate in hard physical activities.
• You are taking any other prescription or nonprescription medications, vitamins or supplements, especially birth control pills and vitamin A.

What are the possible side effects of taking isotretinoin?

Side effects from taking isotretinoin include dry, scaly and red skin; dry lips and mouth, dry eyes and nose, upset stomach, strange taste in the mouth, hair loss, changes in finger or toe nails, sensitivity to sunlight and decreased ability to see in the dark. If any of these side effects are severe or persistent, tell your doctor.

Severe side effects that should be reported immediately include severe nausea and vomiting, yellow skin or eyes, negative mental mood changes such as depression, thoughts of suicide and aggression; joint or muscle pain, abdominal pain, severe headache, rectal bleeding, dark urine, chest pain, changes in hearing or vision, pain when swallowing, and peeling skin on the palms of the hands or soles of the feet.

What if I forget to take a dose of isotretinoin?

If you forget to take a dose of isotretinoin, skip the missed dose and continue on your regular schedule. Never take a double dose to try to compensate for a missed one, and don’t take this medication on an empty stomach.

How do I store isotretinoin?

Store this medication at a temperature between 59 and 86 degrees Fahrenheit. Keep isotretinoin out of reach of children and pets and protect it from light.

What happens if I overdose on isotretinoin?

Signs of overdose on isotretinoin include headache, dizziness, vomiting, flushing, abdominal pain and loss of coordination. If you think you or someone you know has overdosed on isotretinoin, call the Poison Control Center at 1-800-222-1222. If this is an emergency, call 911.

Warnings

Isotretinoin increases your skin’s sensitivity to light. If you have to be outside, be sure to use sunscreen, wear sunglasses and cover your skin. Do not use sunlamps or go to tanning booths. Do not get laser skin treatments, get waxed or do any kind of skin treatments while taking isotretinoin without asking your doctor first. This medication may cause your bones to weaken. Talk to your doctor about any strong physical activity before participating. Isotretinoin can cause birth defects and can be fatal for a fetus. Your doctor will give you a pregnancy test before you start treatment. Since this medication can be hard on your liver, talk to your doctor about how much alcohol you drink and any other medications you take.

Isotretinoin is the generic name of this medication. The most commonly known brand name is Accutane, but his has been discontinued. Other brands include Claravis, Myorisan, Zenatane and Amnesteem.
